If you are a medical doctor you can download the PICF in many languages by using the link display under the section 'Clinical Form' in the analysis order wizard.

If an email address is associated with your patient case (see patient case creation article) then a link to the online PICF will be enclosed in the 'Payment process' email.

If you are a patient and you don't have an email address you can ask your doctor to provide you a printed version of the Patient Informed Consent Form including the instructions to return it to OncoDNA.

If you are a patient and your email address was previously encoded in OncoSHARE, you will be invited to complete the online PICF by using the link enclosed in the ''Payment process' email.
